work-life balance! Our nurses typically work 40 hours per week
Monday- Friday, until 5:00 pm , leaving you available to enjoy your
nights and weekends! ( The Registered Nurse must be available to
travel to assigned group homes throughout the week. Group homes are
located throughout the state of CT but for this position we have
multiple openings in the Greater New Haven Area, (North Haven, East

Registered Nurse ( RN) Responsibilities: Plan and manage care for
individuals with developmental and intellectual disabilities
Provide in-person medical case management to a set caseload of
individuals living in community-based settings Establish and
maintain close relationships with the individuals in your care
Collaborate with support staff and medical team to ensure each
individual has proper medical care Provide ongoing training,
education, and support to a team of support staff Participate in
regular team meetings to collaborate and share information and
experiences Use your nursing judgment to provide valuable insight &
nursing guidance to support staff. Develop plans to ensure that the
medical and health care needs of the individuals are met Generate
written initial, quarterly, and annual nursing reviews for each
individual. Attend all annual Individual Progress (IP) conferences
and other meetings as required. Provide regular monitoring of each
individual's medical and dental records to ensure that appointments
and follow up have occurred as ordered by the healthcare providers.
Complete a monthly review of all orders for each individual.
Ongoing review and documentation of medical or medication incidents
to ensure that the individuals have received medical care as needed
and that the reporting procedure for such incidents was followed by
program staff. Provide oversight, training, and documentation to
ensure that program staff are implementing and adhering to DDNCS
Medication, Nursing Delegation and Medical/Nursing guidelines and
policies. Registered Nurse (RN) Medication Administration
Responsibilities Review the kardexes and physician/dentist order
for each individual regularly. Review documentation to ensure that
certified staff follows DDNCS recommended procedures and DDS
regulations for reporting errors in medication administration.
Prepare quarterly medication administration reports based on the
monthly reviews of the medication record. Complete DDS Quarterly
Medication Audit and submit copies to DDS, DDNCS office, and the
agency. Registered Nurse (RN) Documentation Responsibilities Review
individual records to ensure that program staff are correctly and
adequately documenting the medical treatment of individuals. Ensure
that documentation of seizures is maintained in a format that will
allow for at least three years of information as required by DDS
regulations. Registered Nurse (RN) Training Responsibilities
Provide LPN oversight as required by DDS regulations. Provide New
Employee Training at the assigned agencies/programs. Supervise the
initial worksite administration of medications by certified staff
and document the supervision/observation required by DDS
regulations. Conduct onsite training and retraining for staff as
needed as required by DDS Regulations. Conduct additional relevant
training as assigned by DDNCS. Requirements Registered Nurses must
be available to travel to assigned group homes throughout the week.
Group homes are located in the greater New Haven Area. Active RN
license in the state of Connecticut Valid driver's license and
reliable transportation for daily commute to residential
facilities. Experience working with individuals with developmental
disabilities 2-3 years of experience in nursing preferred Covid
