    The Palliative Care -Social Worker (LCSW) will plan, organize, and implement social work services to patients and their families to assist them in meeting social and emotional problems arising in connection with illness or hospitalization.

    Key areas of responsibility


    • LCSW performs/documents psychosocial assessment
    • Supports the palliative/health care teams by facilitating/leading goals of care conversations and advance care planning with patients/families
    • Assists the Palliative Care Team in ensuring excellent patient care services, coordinating educational and outreach services, participating in quality improvement initiatives, and collaborating with other hospital departments, and outpatient and community services
    • Serves as a liaison with the hospital hospice and other community hospices, home health, and nursing homes to ensure care coordination and appropriate assignments and resource allocation to meet patient needs throughout the continuum of care
    • Coordinates palliative team efforts with appropriate hospital support departments to ensure smooth discharge planning and clear follow-up post discharge; may assist in the development of creative community or system care options for patients
    • Supports and participates in activities that promote the growth of palliative care services into the community and outpatient settings
    • Participation in the coordination and delivery of bereavement activities and services as well as other related duties as required

      Qualifications Education Requirements


      • Master's degree in Social Work (MSW)


        Minimum Requirements


        • Licensed Clinical Social Worker (LCSW) in the state of CT
        • Minimum of one year of casework experience highly preferred
        • Acute care experience desired
        • Excellent interpersonal and communications skills
