utilizes their extensive education and training to provide
personalized care to our patients in a rewarding environment that
promotes autonomy. Job Description - Support our patients,
families, and loved ones, as they go through their end of life
journey - Perform a comprehensive assessment to establish goals for
symptom management and end of life care with effective case
management strategies - Be a member of an active interdisciplinary
team that addresses patients’ needs holistically - Develop and
maintain a therapeutic relationship by utilizing effective
communication with patients and their families. - Utilize
state-of-the-art technology to document skilled services provided.
Qualifications Registered Nurse with Valid CT RN license - One year
of RN experience; Homecare or Hospice experience preferred. - Valid
driver’s license and reliable transportation required. - Computer
