Please list any anticipated procedures/surgeries the provider will be required to perform and anticipated volumes: C-sections D&C Description of Work Assignment: In house OB coverage Breakdown of Clinic Days/Sessions and/or OR Days:24 hour restricted call shifts Schedule of Days/Times Needed to Work/ Shift Hours flexible On call? Restricted call Patient Load 3-7 deliveries per day triage patients Board Status Requirement (BC/BE) BC/BE Is there additional support staff? L&D staff Is the provider working solo or with other providers? If so, whom? Solo Are there Advanced Practice Providers? If yes, is the locums provider responsible for them? Yes. CNMs work with provider at times in L&D Trauma Level of Work Location EMR System? EPIC Is the Clinic EMR Different from the Hospital EMR?
