Job Description Job Description We are seeking a dedicated Registered Nurse (RN) or Physician Assistant (PA) to join our Population Health Division. The ideal candidate will have experience or strong interest in value-based medicine, high-risk patient management, and addressing gaps in care. This role plays a key part in improving health outcomes for marginalized and underserved populations, supporting our mission to deliver equitable, high-quality care. Key Responsibilities Monitor, track, and close gaps in care across value-based programs. Conduct outreach and follow-up for high-risk patients to support care compliance. Review medical records and clinical data to identify required preventive screenings, chronic disease management needs, and overdue health measures. Collaborate with providers, care coordinators, and the quality team to improve population health metrics and clinical performance outcomes. Support patient education on chronic disease, preventive care, and care plan adherence. Assist with the development and implementation of value-based care initiatives, including workflows that improve quality scores and patient engagement. Participate in quality audits, documentation reviews, and reporting for payor-based value programs. Promote health equity by helping remove barriers to care for underserved patient groups. Qualifications Active RN or PA license in New Jersey (required). Experience with value-based medicine, quality measures, or population health strongly preferred. Knowledge of HEDIS measures, preventive screenings, chronic disease management, or care coordination is a plus. Strong communication, assessment, and patient engagement skills. Ability to work independently, manage follow-ups, and prioritize patient needs. A passion for improving health outcomes in marginalized communities.
